Recently my disapproved items went up to 21% (272 items) and we didn't change anything.

I was looking at some of the errors, the following were 404 not found :

http://northseaworkwear.com/blundstone-548-crazy-horse-leather-rancher-boot-brown    item not listed on live site
http://northseaworkwear.com/niagara-ba8871-brown-mens-boots-by-amblers            item not listed on live site
http://northseaworkwear.com/blundstone-548-crazy-horse-leather-rancher-boot-brown    item not listed on live site

http://northseaworkwear.com/dunlop-purofort-c462840-wader-green                live on our site
http://northseaworkwear.com/sip-forestry-jacket-1skp                    live on our site
http://northseaworkwear.com/snickers-workwear-3314-trouser-blue                live on our site
http://northseaworkwear.com/sip-chainsaw-protection-jacket-1sj9                live on our site

I checked and 4 were live on the site and the other 3 were not listed - if we don't list them then the feed shouldn't be looking for them ?

generally, verify the following:
(1) there is only one, single, feed registered for the target country;
(2) there is only one live feed for the target-country in the data-feeds tab;
(3) the one registered feed is only registered once and never deleted;
(4) all id values are unique and id values never change for any item;
(5) the web-server, website, and web-pages adhere to all google policies.

after the feed is re-submitted google will regularly re-crawl items,
images, and the entire site for policy and quality related issues.

there may be a delay of 72-hours or so between feed updates and
crawls; so, be certain the current, live, link url for each item listed within
the merchant-center, return a proper (200 ok) response, exactly match
the website landing-page details, and return the exact same response
and content for all users, all web-browsers, at all times, in all locations --
regardless of where the user is physically located.

check the frequency and rate of feed changes,
feed re-uploads, and changes to the website.

testing within a single local browser is usually not enough to verify what
google requires in terms of content, response, and quality -- rather, check
the web-server logs for a proper response to all google's crawls, and check
the link landing-pages from within several browsers, using incognito/private
mode, from several locations (or remote proxies) if possible.

importantly, google may only list a subset of issues --
the root-cause may effect many more items long-term.

too many errors or too frequent errors can result in items removed from the
listings, disapproval, or (permanent) suspension from the program, at any time.

currently, for me, those live-on-our-site
items are also returning a 404 response.

unfortunately, forum members cannot look into feed or account histories --
the best likely course would be to carefully inspect the merchant-center
account and server logs; then, work with whoever is currently supporting
the site and google directly to find the root-case and take corrective action.

I see. I loaded them up in Internet Explorer but I got the 404 response in Firefox and Safari

that does not seem at all typical or proper behavior for a website --
and something that google would likely flag enmasse eventually.

the web-hosting, web-server, web-site, e-commerce,
and web-page design, configuration, and development
should likely all be considered as possible suspects.
