Help needed - web sales conversion tracking[ Edited ]

I am looking to use Google Analytics to set up e-Commerce tracking for my organization's membership sales conversions. The transactions are made via my organization's new website. We would like to measure how effective the website is at converting traffic into new and returning memberships.
Specifically I would like data on the number/ratio of people landing on our pages that proceed to the order form and then click the button to complete a sale.
I am including a link to the membership application area of the website give you a visual. Note there are five steps to complete the transaction.
Any help to set up this tracking would be most appreciated.

If you want to track checkout process on your site by e-Commerce tracking, you can implement Enhanced e-Commerce (https://goo.gl/BfjmhA) plug-in for analytics.js. It would be helpful for you to use example of implementation (https://goo.gl/ZG8eRs).
