Goal Tracking in Analytics

I have created a goal that I want tracked: "Request a Demo". It is supposed to track people that submit our demo form (that is iFramed on the page).

Unfortunately, that is not working. Attached is a screen shot of what I see when I "inspect" the page and submission in Google Chrome:2015_01_-_Google_Analytics_Request.png

Our form is hosted by Pardot. I have had them look at the coding on the form and the template and they have not found anything wrong.

Can you please help?

Thanks in advance!

Here is the code on the Form's thank you content:

<script type="text/javascript">
try {
  ga('create', 'UA-228099-1', 'auto'); ga('send', 'pageview', '/thankyou'
} catch (err) {}

Here is the code that is on the Form's template:

<!-- Google Analytics Universal Tracking Code -->
  (i[r].q=i[r].q||[]).push(arguments)},i[r].l=1*new Date();a=s.createElement(o),

  ga('create', 'UA-228099-1', 'auto');
  ga('send', 'pageview');

The Goal details in Google Analytics are:

You can start with basic debug on the website with GA debugger:

You can check in chrome, console tab with 'Preserve log' turned ON.

GA Guru.

'Star' or 'Accept as Solution' if it helped.
