GA skipping on some goals in the report

Our GA has been skipping some of the goals(conversions) in the report. For example, we would receive 4 leads, but the goal report only shows 2 or 3 conversions. They all come from the same webpage, which is a lead gen form. The goal should be recorded when the user enters the funnel by filling out the form, submits the form, and reaches the Thank you page.

 

What's odd is that not all goals are skipped, but has been quite often recently.

 

Is there anything I can check to see if the settings are off?

Hi,

 

It can happen that the user submits the form twice and reaches the thank you page twice in the same session. In this case only a single goal completion will be recorded. Hope this helps.

Hi Arnold,

 

Thanks for your input, but the forms are all different from each other. So Far, I've recorded 9 submissions this month, yet GA only shows 3.
