GA shows wrong location of users who made some actions

Hi there.

We are facing problems for last few months: GA shows wrong locations of users in Overview table, section Conversions.
All users that hit buttons on our website are shown as users from Ukraine. However I do know they are from other countries. For example on February 22 at 4.39 pm a guy from Singapore hit the Send button on my website (we took his IP from server logs http://ipaddress.is/115.66.115.245#.WK25NRJ950s), but GA shows he is from Lviv (Ukraine). The same happens with users from London, US, every other country - all of them are displayed as users from Lviv.
Moreover, some actions are not displayed at all in Overview table. But still I can find them in Events section.

Do you track button clicks as events on clients side? Or have you some other implementation? I've made some queries on your site and didn't noticed any specific click data being sent.
