Associating a form submission with the referral type

I am running several Facebook and PPC campaigns. I am able to track each ones conversion rate for visits to my website, but how do I associate a lead that has been submitted in a form to each campaign.

I have a form that asks for a name and email address, i would like to know where that lead came from.

How do I associate a form submission to a specific source/medium etc?

Thanks

Hi,

If there is no separate thank you page after the form submission,

you could add event to track them.

I don't know your setup but let's say you have a piece of code that displays Thank You message. Within that code you would execute:

ga('send', 'event', 'forms', 'submitted', 'success');

Then you can set this event as a goal which gives you the possibility to report all sorts of things.
You can add Secondary Dimension in Event report with source/medium if you do not want set it as a goal.

Let me know if you need more advice

Hey thanks for your response

I do have a piece of code that displays thank you after submission. So I
would add the following to that piece of code?

ga('send', 'event', 'forms', 'submitted', 'success');

Do I put this above or below that code?

What we want is to be able to associate a lead with a specific marketing
channel?

Is there a way to add a hidden form field that generates this source/medium
when submitted?

Thanks

Just make sure that GA piece of code is executed only when thank you message is displayed. You can use tag assistant extension or real time reports to confirm that after you have published it.

GA has the source/medium already for the session within event occurs so no need for any additional configurations

Would I manually add this piece of code to my site or can I implement this
through tag manager?

Thanks so much
