Weird Source / Medium in stats (eg. google / (none) or site.com / (none))

Hi there!
I've got some weird statistics (in my opinion). I can see separated records in Source / Medium ("site.com / referral" and "site.com / none"). Moreover that "site.com / none" has a close to 100% Conversion Rate (btw, what is it). That "google / (none)" - what is this? Also: in Audience - Technology - Network - Hostname I see my domains separated to www and non-www. Non-www conversion rate is 0%, and www is up to 100%. What is this?
P.S. I don't know why (it wasn't me) Referral Exclusion List.Referral Exclusion List.Referral Exclusion ListReferral Exclusion Listnon-www version is in Referral Exclusions list now - maybe this is a reason?

Btw, the goal isn't in the site's code since it is a CallRail "Phone" event.
