Campaign Source and Medium not showing properly

Our campaign reports are not recording Campaign Source or Medium.

 

We are using the following URL:

Is the URL incorrect or if there something we should check on our GA setup?

Are you sure you're looking at the correct property? (UA-59206892-1)
If so, can you move your GA tracking code up into the head tag (it's supposed to be there anyway) : https://support.google.com/tagassistant/answer/3059154?ref_topic=2947092#move_head

Can you check your real time reports to see if the visit is being logged? Are you sure that you didn't just exclude yourself and the tests are not working because you're deliberately not tracking traffic by yourself?

The URL itself looks to be set up perfectly.

By the way, I just visited so if you are blocking yourself using a filter or whatever, I should at least have shown a couple of page views. Let me know.
