How to filter out two IPv4 addresses and one IPv6 in one filter?

I have two different IPv4 addresses and one IPv6 address I need to filter out of my GA view. I found directions on how to to the IPv4 together, but I'm not sure how to include the IPv6. Can someone tell me the steps and an example of how it should be entered in? Thanks!

There is an example on the following page of how to setup a filter to exclude IPv6

You can create and apply more than one exclude filter 

Problems usually occur when it comes to include filters for the same filter field. 
Simply because if the hit does not fulfill the first filter condition, it gets discarded
and doesnt make it thru for evaluation by the 2nd include filter.
