What do I really need to put in my URL?

The URL builder has always been a bit confusing to me.

eg :I send out a newsletter with one link back to my website.

Do I need to put medium or even campaign name? I see "Newsletter" and I know the medium is email. I've read a lot of articles that advise taking advantage of these fields, but I'm wondering why I need to fill in info that seems redundant. 

 

I'm also not sure what the hierarchy is with these fields. 

eg: I'm tracking traffic from withing my website. Three pieces of info I want to track:

"my_website" is the top level (the user is on my website)

"x_page" is the next level (the page the user is on)

"x_link" is the bottom level (the link the user clicks)

What fields would these pieces of info go with?

 

If I tag all my newsletters, order confirmations, etc. with "medium:email," can I sort by medium in GA to see all of my email data?
