Same web property ID is tracked twice - What this mean?

When I'm checking my website with Google Tag Assistant Chrome extension.
It's showing in Where To Optimize Section:
Same web property ID is tracked twice.
Tag not fired through GTM
-- Can't understand what's the actual error is? - Please help.

Hi :-)
As per your screenshot.. in green it shows 2 pageviews being fired for a single page
and as per the yellow "where to optimize" it states the same property id is being tracked twice
and that at least one of those tags is not being fired by GTM . 

Looking at the site source code via browser-
GTM is installed (and we know this contains a GA Tag for property UA-16603780-3)
There is also a copy of the GA tracking code snippet on the website for same property for UA-16603780-3.

Need to remove the copy of the GA tracking code
