Debugging missing tracking code | Multiple Google Analytics tags detected

I did a simple integration of GA to track page view.  Our web app is a single page application (SPA) and thereby I used following code,

if(window.location.hostname && (window.location.hostname === "dev..")){
googleAppId = "dev_code";
} else if(window.location.hostname && (window.location.hostname === "qa..")){
googleAppId = "<qa_code>";
} else if(window.location.hostname && (window.location.hostname === "www..")){
googleAppId = "<prod_code>";

(i[r].q=i[r].q||[]).push(arguments)},i[r].l=1*new Date();a=s.createElement(o),
ga('create', googleAppId , 'auto');

For other view, I have written the following code in controller so to track each view click,

ga("send", "pageview")

The above is the GA setup.

However I am getting the below error for around a month,

Missing Tracking Code
No valid tracking code found for property <prod> default URL http://<prod> Make sure your pages are tagged with a supported version of the tracking code.

I also installed Google Tag Assistant Recordings, which often suggest to optimize the code Multiple Google Analytics tags detected.

The reason is we are using 2 third party widgets,

1. instansive

2. disqus

Both have their own GA integration. Both are iframe so their GA code is not editable in my side.

I am totally confused.  Please shed some light on this issue.

Thank you,
