Payment Unsuccessful

This situation has me completely at a loss.  After successfully running ads from 1/16 through 2/22 and making automatic payments from the same bank account, my account was hit with a suspension for suspicious billing activity.  It might also be worth mentioning that my account was suspended with 0 disapproved ads in any campaign due to being able to chat with various Google chat agents who reviewed and helped my landing pages/ads get approved.  Unconvinced that my account had violated any policies, I submitted the email form for suspended accounts and the follow-up appeal form. Nevertheless, my appeal was unsuccessful and so were my efforts for phone support. I'd really like to just put this all behind me, but my account has an unpaid balance below threshold and all of my attempts to pay have been unsuccessful. My bank told me that it isn't them who's declining the charge either. What can we do?

Hi there;
What was the reason for suspension mentioned in the email received from Google? (Look for this email - it is the key for understanding the issue)
Please share the URL of the site suspended.

This is the first email sent to me by Google informing me of the account
suspension:

Hello,

We've identified suspicious behavior in the payment activity of your
AdWords account or a connected account. To help prevent any unauthorized
activity, we suspended your AdWords accounts and stopped your ads from
running.

Here is the second email after appealing:

Dear Advertiser, Thank you for your email.

Thanks for your continued patience during this process. Our specialists
have re-reviewed the account and confirmed that it is in violation of our
AdWords policies. Since this decision is final, the account will not be
reinstated. Please avoid creating additional AdWords accounts, as they will
be subjected to the same suspension.

Any advice on how to improve would be appreciated. If not, I'd like to be
able to pay the remaining balance and close the account.

Hello again;

My view: your site violates the policy and your business model cannot be promoted on Google; 

There are several reasons; . The main one; the primary goal of your site is (as seen by a user) is to collect personal information (i.e. an email)

- This business model is not allowed.

Hey MosheTLV,

Thanks for your feedback. After fully taking your advice into
consideration, I went over my site and removed the contact form from my
landing page, because email collections are not the primary goal of my
business, and I added a few legal pages that I felt were missing. What's
odd is that I received an email yesterday from Google informing me that my
ads are able to run again, however my account is still suspended.

Some updates were made to my site after our last conversion.  Primarily, the email form was removed.  Are there any other considerable changes that need to be made that would allow me back on the network?
