I need to change this ad for a new client. What is above the sitelink extensions and previous visit extensions? Is this something that I can generate or did Google bots scrub this and insert?

Those are from the call-out extension, which can exist at ad group, campaign or account level.

 

It is also possible that they are automatically generated. You see them in the Ad Extensions tab, it is the bottom choice from the drop-down box. They can be switched off, or if you create call-out they will replace them.
