I've been told some ads for religious institutions will be approved despite the restrictions on religious advertising. Does anyone have examples of what has not been approved? Is it mainly that keywords associated with the ad can't be "belief" based?

To the best of my knowledge AdWords does not have a general policy restricting religious advertising, however, two principal regulations apply.
*** Inappropriate content ***
It is sexual/religious/or political intolerance that is not supported. ("Examples: Advocating against a person or group on the basis of gender identity, sexual orientation, race or ethnicity, disability, religion, or age.")
*** Personalized advertising ***
Additionally, if you are in for personalized advertising (formerly known as interest-based advertising), you are not supposed to target users based on their identity and belief. The scope of this policy is as follows:
- affinity audiences
- custom affinity audiences
- in-market audiences
- similar audiences
- demographic and location targeting
