1. How does many per click actually work?

IP, Cookie what exactly matters?

 

2. if a user comes on 29th day through bookmark and makes purchase will the cookie be refreshed or new cookie will replace the old cookie?

As I said above, Adwords cookie would expire 30 days after the user's click. Hence if someone completes the sign up form or a purchase after that, Adwords won't record it. See the below screenshot taken from hel center:

I think you are referring to Conversions (many-per-click) here. Now as per Google, it's the cookie which does the trick and your Adwords account records conversions (many-per-click) or 1-per-click depending upon how they actually work.

 

Conversion tracking cookie has a 30 days window and if the user has clicked an ad within that time period, Adwords would record the conversion and increment the respective values of 1-per-click and many-per-click. Within these 30 days if multiple conversions have ocurred from that single click you will see more values for many-per-click and only unique values for 1-per-click.
