Serve Call Extensions by Keyword?

We have ad groups based on legal services, each service may have two or three providers each with a different phone number. There are over 1,600 providers across all of the different services.

 

An example: In an ad group for 'Family Law' where the keywords are the lawyer's names, how can I serve the correct call extension phone number that matches the correct keyword (name)?

 

The ads use customizers from a business data feed and the feed has the match between name and phone number.

You can only add extensions down to the adgroup level of the account. So you have to have a different adgroup for each provider, and apply the correct phone number to each adgroup.
