Is there a way to tell if my ad is getting clicked on maliciously?

My ad is getting clicked, yet I am not getting calls which is quite out of the ordinary.  Do I get charged for each click, if a single IP clicks my ad multiple times?

Hello, Sean and welcome to our community.

Unless AdWords' algorithm, which is fairly complicated, detects a malicious behaviour pattern, you do, indeed, pay for those clicks.

However, if you examine your webserver's logs and find out that you do have a lot of clicks in a short timespan, all from the same IP, you can report that to Google via this form: https://support.google.com/adwords/contact/click_quality
