Form filling display ads- How to run it?

Hello There !

 

I came across a display Ad,featuring in-banner form filling fields, meant for visitors to furnish the contacts ( email, tel, no etc). This particular one is aimed to collecting leads of viewers  interested for test driving of car brand.

Kindly see the below screen shot of Ad.

 

Can some one guide on;

How this can be run?

What is googles term for the Ad unit?

Can it be run through Ad Words?

and how to know about it more on targeting and placement and restrictions etc?

A form cannot be made functional in AdWords ads technically. Even if someone creates an HTML5 ad with the form in it, it won't serve the purpose. The reason being data fields in the form will not be carried over to the landing page. And thus, it will defy the very purpose of having it.

 

And yes, as @MosheTLV said, it will most likely be violating the AdWords policy. 

 

However, such ads can be created and are supported by DoubleClick platform where Rich Media Ad Formats are supported. 

 

An example can be https://www.richmediagallery.com/tools/template-database#tplDbCollection:10813~

To my recollection those   ads (aka "lead form") are  not supported by AdWords (because   of Policy which does not allow to collect  PII on the ad)  but by doubleclick.

Though, I have not found them on the rich media gallery;

Can you please share how double click platform can be accessed to run similar campaigns? Basically would like to know if we can access this similar to Ad Words account or not?
