All calls are showing as missed calls
In my Google Adwords account from 19th October, 2015 all calls are showing as missed calls.
There are two possibilities behind this issue:
1. If any end user has a call and/or anyone does not receive the call than this become as a missed call.
2. If any end user has a call and the call gets dropped.
*But these two possibilities have not with my adwords account.
So, what is the issue in my adwords account ( call tracking ).Please take a look. Please let me know if there are any suggestions.

Were your calls being recorded normally before the 19th?
There's a few reasons why this might have happened and I've seen this before.
1) Users are not expecting to automatically call the number when it's clicked and they hang up immediately (an issue I have with this format in particular).
2) The forwarding (Google Voice) was not working or the number on your end was not working. This is the most likely reason.
Have you tested this yourself? Can you get through?
It is strange that all were considered missed given the variety of sources and source area codes.
Can you share your screenshot when you click on "manage conversion actions"?
