Suspended Adwords Account - Policies Reviewed

Well it seems I got the red bar of doom when I tried activating my campaign. I've read the policies prior to posting this but still not sure what the issue is.

Red Suspension MessageYour Google AdWords account has been suspended because your billing information in this or a related account could not be verified.

 

First Suspension Email: While reviewing your account, we found violations of our AdWords policies  
in this or a related account. As a result, your account has unfortunately been suspended, and your ads will no longer run on Google.

 

2nd Suspension Email: We periodically review accounts for security purposes and to verify billing information. As a result of this review, your account and any related accounts have been suspended, and your ads are no longer running on Google.

I had previously entered the billing information before finalizing my campaign and everything seemed fine. Within minutes after I made the campaigns live the red bar of doom appeared with the message as stated.

 

AdWords Setup:

3 Campaigns, 20 keywords Each, 4 Sitelinks each, 1 Offer extension, Call Extension  

 

3 Landing pages were created specifically for each of the 3 ads & landing page sitelinks submitted via Google Webmaster:

Ads were approved.

 

Policies Review & Possible Issues:

2 Sitelinks & Offer Extension were dissaproved. - I edited the sitelinks and removed the Offer Extension to conform with Policies. Resubmitted campaign and received the same policy violation email as before.

 

Possible Arbitrage Issues:

Landing pages created for ad campaign may be short on content and not listed in menu.

Landing page contains a link to a financing company we use for clients.

Website does contain images linked to supplier.

 

Possible Technical Issue, Mirroring & Framing:

We use a CMS for the website. 

May have a mobile site missing the rel="canonical" tag causing duplicate issue.

 

I have not received any kind of indication as to which policy we have broken. Attempts to phone customer support failed due to suspended account. I'm just not sure if any of these reasons I've listed would cause a suspension.

 

Any help you could provide would be GREATLY appreciated.

Hi MosheTLV, I filled out the form again (this is the 5th time I've filled out that form), so hopefully I will get confirmation on exactly which policy we violated so I can remedy it. Any assistance you can provide would be greatly appreciated.
