Ad disapproved for 'Unidentified business'

I have for image ads all going to the same landing page. They all name the product and the landing page clearly has the product on it. Three of my ads are approved and one is not approved for business unidentified reason. I am confused how to make any corrections.

The AdWords Editorial policy requires your ad to have the product or company name as the last static image, and that the ad itself clearly displays what is being advertised. Nobody has been clear on what exactly do we need to do to fix this. Please do not refer my to another blog/policy.

Watermelon W is exactly right. Has there been any update on this? We know what the policy is - the problem is AdWords disapproving ads inconsistently (such as in Ben T's situation).
